Every situation is different, every investigation is unique and every academic or operational endeavor requires a focus on the problems on hand and not a feel-good program to make your organization feel safer.

We understand and firmly believe that error counting, time since last event and other metrics commonly employed in many industries are just that. Feel good programs that do not actually try to understand the complexities of our social-technical systems. Classification and error categorization are no longer helpful in understand the emergent properties of these systems.

No single act causes safety and no single act causes a catastrophe. Spending your safety department time chasing these numbers and fitting sharp-end employee actions into neat categories will not reveal your true margin of safety.
